As even small lifestyle changes can have a major impact on a person's health and, subsequently, on the costs that he or she incurs, the idea is to take a more patient centric view of care. By involving the patient directly, they are more likely to be motivated to take action rather than being told what to do, which in turn will contribute to reduced hospitalisation.
Patient data
With Sonetto® Healthcare, you can have a single integrated view of the patient, from basic information, to medical history, diagnosis and treatments. Providing a single view of information allows for more complex algorithms to be run and build more accurate recommendations for treatment.
The information can be entered directly into Sonetto®, but also can be loaded directly from external systems, including PCT databases or local healthcare applications as required using the existing components in Sonetto® iTransform.
Patient scheduling
To ensure that the patient receives the correct recommendations for treatment and that prescriptions and treatment plans are being followed, Sonetto® Healthcare includes a workflow and scheduling component. This sends notifications and updates to patients and clinicians either in the form of a printed letter, email or appointment. A record and history of communications are kept for each patient.
Security and Data protection
Sonetto® Healthcare has a comprehensive security system, using a rights and roles concept for granting access. Generally, access to patient information will be strictly limited, However, where it is required, anonymous bulk access can be given for reporting, such as is required by research assistants.
